The support team won't review your custom javascript or HTML, but can review the setup of form fields, mappings, etc., in your CRM. However, in going back and reading your description of the problem, it sounds like the Posted Form is getting linked to a new Lead, but it is the Visit that does not? Is that correct? If so, that would be the normal behavior in this case because the visit is created before the form is submitted, and therefore is created before the visitor is identified. Under normal conditions when a real person is browsing a site as an anonymous visitor and then fills in a form, their previous visits should get updated to be associated with their new Lead record. However, in this instance, it is likely a sort of race condition because you are generating your own visitor id and submitting the form in rapid succession. The Visit and the Lead that results from the Posted Form are created separately, so it could be that the Visit isn't available for the visitor key to be referenced when the Posted Form is created and the new Lead is created.
