Is it possible to create a page (general landing page) where a user could supply his/her email address and then click a button that will take the user to a form that is pre populated with that users contact/lead information?

So the user should be identified with the email address to se if it is an existing contact in CRM. And then he should be able to click a button/link that would take the user to a pre populated form.
